Hydroxychloroquine 200 mg – An Antimalarial Drug

Description

Hydroxychloroquine is a medication prescribed to treat or prevent malaria. It is also used for the treatment of autoimmune diseases such as rheumatoid arthritis and lupus. Sold under the brand name Plaquenil, Hydroxychloroquine belongs to a family of drugs called 4-aminoquinolines, or antimalarial drugs. It is a close cousin of chloroquine, which is also an antimalarial drug. The drug also belongs to disease-modifying anti-rheumatic drugs (DMARDs). It can provide relief from the skin issues caused by lupus ad prevent pain/swelling of joints in arthritis. The U.S. Food and Drug Administration (FDA) approved hydroxychloroquine for malaria prevention and treatment in 1955.

Action Mechanism

Hydroxychloroquine increases pH within cells and alters certain cellular processes. The exact mechanism is unknown but it is presumed that the drug interferes with antigen processing in the cells. The drug diminishes the formation of some protein complexes that are required to stimulate immune cells. Some studies have found that hydroxychloroquine has strong antiviral effects by preventing the viruses to enter the cells.

Dosage

  • For proper dosage and directions, please check with your primary care physician
  • The drug is available in 200 mg potency, which is to be taken orally with a glass of water, with food.
  • For malaria prevention, you may need to take hydroxychloroquine 400mg once a week for two weeks before the exposures.
  • For malaria treatment, 800 mg followed by 400 mg at 6 hours, and then 24 hours and 48 hours after the initial dose.
  • For lupus, the recommended dosage is 200 to 400 mg daily; dosages over 400 mg a day are not advised.
  • For rheumatoid arthritis, the initial dosage is 400 mg to 600 mg daily.
  • Take hydroxychloroquine at least four hours before taking your antacids or diarrhea medications.
  • Take the medicine at the same time(s) each day if you are taking it for lupus or rheumatoid arthritis/
  • Make sure you take this antimalarial/anti-rheumatoid medication exactly as prescribed and avoid overdosing.

Precautions

  • Before taking hydroxychloroquine tablets, it is imperative to inform your doctor if you have a history of drug allergy, including chloroquine.
  • Also, tell your doctor about the medical conditions you have or have had, especially G6PD deficiency, eye problems, kidney or liver disease, alcohol abuse, psoriasis, or others.
  • More importantly, tell your doctor if you have diabetes because you may need to check your blood sugar levels during treatment.
  • Inform your doctor if you are taking any other medications before starting with hydroxychloroquine treatment.
  • The drug may cause mild to moderate dizziness, so avoid outdoor activities during the course of therapy.
  • Before using this medicine, tell your doctor if you are pregnant or breastfeeding.

Side Effects

Some of the common side effects of Hydroxychloroquine include:

  • Nausea
  • Vomiting
  • Loss of appetite
  • Diarrhea
  • Dizziness
  • Headache

If these side effects persist or aggravate, check with your doctor immediately. Most people using this hydroxychloroquine do not experience serious side effects. Seek medical attention right away if you experience serious adverse events such as palpitations, mental/mood changes, easy bruising/bleeding, severe abdominal pain, muscle weakness, or extreme dizziness.

Warnings

  • Hydroxychloroquine may increase the risk of a heart condition called arrhythmia if you already have cardiovascular disease. So, seek medical attention if you experience palpitations.
  • The drug may also make your skin more sensitive to the sun, so limit your exposure and avoid tannings booths or sunlamps. Make sure you use protective clothing when you go outdoors.
  • Avoid alcohol or cannabis during the course of treatment. In addition, alcohol may worsen your liver condition, while you are on hydroxychloroquine treatment.
  • Do not exceed the dose prescribed by your doctor because an overdose could increase the risk of adverse effects.
  • The elderly may be more vulnerable to the potential side effects of this antimalarial drug, especially those who have heart disease.
